Complaints

Occasionally you may be unhappy about something that has happened or about your treatment. If this is the case please feel free to email us at Reservoir.roadsurgery@nhs.net. Where our Practice Manager, Yasmin Kouser, will reply back to you as soon as possible. 

We will then endeavour to put the matter right. If you feel that this has not satisfactorily resolved the problem, you can approach the Parliamentary and Health Service Ombudsman to review your case (www.ombudsman.org.uk).

You may come into the surgery to collect a form, or you can print out the form linked below. We expect written complaints, if its needed to be discussed in person, we will arrange a meeting best suited to you.
