We register new patients every Tuesday and Thursday Afternoon between

2.30 pm and 4.30 pm

If you wish to register with our practice we require:

  • Your NHS number and details of your last doctor. This information is on your Medical Card if you have one
  • Proof of your name, date of birth and current address (eg passport, driving licence, birth certificate, rent book)
  • If you are newly arrived from a country outside the EU we need to see your passport and visas (originals only, copies are not acceptable)
  • At the time of registration we will make you an appointment with our Practice Nurse for a health check. We will also ask you to complete a questionnaire; this will help us to know a little about your medical history and current medication until your notes arrive from your previous doctor. It is important that you keep this appointment.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Residents

If you are away from home and in need of urgent medical attention please contact the nearest doctor - ask the people you are staying with, who they are registered with - you will be seen as a temporary resident. Equally if you have friends or relatives staying with you, we will be happy to see them as temporary residents, but there may be a charge if they are from a country with there is no reciprocal healthcare arrangement. Ask reception for details.

Please let us know as soon as possible if your address changes.

National Data Opt Out (The deadline is now 30 September 2021)

NHS Digital will begin data collection from 1 July 2021. Data may be shared from the GP medical records about:

  • any living patient registered at a GP practice in England when the collection started - this includes children and adults
  • any patient who died after 1 July 2021, and was previously registered at a GP practice in England when the data collection started
